• SF-3107 Application for Immediate Retirement under FERS
• OPM 1515 Military Service Deposit Election
• SF-2818 Continuation of Life Insurance
  You are required to sign and date this form in four separate areas
  (after blocks 8, 9, 12, 15)
• RI 38-124 Voluntary Contributions Election (FERS Transferees Only)
• W-4P Federal Tax form

Once forms have been completed on line, print, sign and date. Mail the completed forms to your Benefits Specialist.
