• OPM 1515 Military Service Deposit Election
• SF-2818 Continuation of Life Insurance
  You are required to sign and date this form in four separate areas
  (after blocks 8, 9, 12, 15)
• W-4P Federal Tax form
• RI 38-124 Voluntary Contributions Election (FERS Tranferees Only)

Memorandum: You will need to complete this memorandum found at the link below if you are retiring after the last day of the month. It is a required part of your retirement package.
