Forms: You will need to complete the forms found at the links below that apply to you. The forms are required by the Office of Personnel Management in order to prepare your retirement package.

• SF-2801 Application for Immediate Retirement under the CSRS
• SF-2801-2-1 Attachment - Spouse's Consent to Survivor Election
• SF-2818 Continuation of Life Insurance
  You are required to sign and date this form in four separate areas
  (after blocks 8, 9, 12, 15)
• OPM 1515 Military Service Deposit Election
• RI 38-124 Voluntary Contributions Election
• W-4P Federal Tax form

Memorandum: You will need to complete this memorandum found at the link below if you are retiring after the 3rd of the month. It is a required part of your retirement package.

• Mid-Month Retirement

Once forms have been completed on line, print, sign and date. Mail the completed forms to your Benefits Specialist.
