Submitting a Nomination

*FY07 Deadline for awards submissions: 07/20/2007* (all monetary award nominations must be delivered to your servicing WFMO Advisor by 08/01/07 in order to use FY07 funds).

  • Only supervisors may submit Special Act Award nominations.
  • Nominations must be made on the CD-326lf, Recommendation for Recognition.
    • Block 1:
      Insert full name of employee - do not use nickname or initials (if a group of employees, type "See Attached" and provide a listing of full names).
      Block 2:
      Please leave blank. DO NOT INSERT EMPLOYEE'S SSN.
      Block 3:
      Insert recipient's organization (Ex. - NOAA/OAR/PMEL)
      Block 4:
      Insert accounting information (code out of which award money comes)
      • 4c. Fiscal Year: FY out of which award is paid
      • 4d.Project: 7 character CAMS project code - Ex. - 52NS1GL
      • 4e.Task: 3 character CAMS task code - Ex. - P00
    • Block 5:
      Insert dates of accomplishment(s) (Ex. - Jan. - Mar. 2004)
    • Block 6:
      Check box in front of "Special Act or Service Award" and indicate amount on blank link to right
    • Block 7:
      Insert narrative or award justification
    • Blocks 8 - 11:
      Obtain appropriate signatures
    • Submission:
      Make a copy and forward the original to your servicing Workforce Management Office Advisor
       

Justification (Narrative)

The justification need not be extensive, but adequate documentation might include:

  • what is significant about the achievement;
  • the scope and extent of the achievement, and the amount of savings, if any;
  • clear indication of how the achievement exceeds normal performance expectations;
  • impact of the achievement on work, goals, and/or mission of the organization;
  • magnitude of the achievement in relation to the employee's job responsibility (degree of ingenuity reflected); or
  • period of time covered by the award.
